Hypnosis and Hypnotherapy, what is it?
Many different people, including hypnotists, make all sorts of wild claims about hypnosis, with some alluding to strange, maybe esoteric, powers of the hypnotist but my research suggests that it is not what you think it is! So, what is it? Some say we experience hypnosis every day, which is, partially but not exactly true, at least not in the context of what they mean! However, it is similar to something we experience daily, something we refer to as, trance states. However, these everyday trance states are usually quite light, mostly transient in nature and completely involuntary. These everyday states, that are perceived as being hypnotic, are somewhat akin to mild sleep states and occur naturally but are not the same as the hypnotic state! During sleep, the neurological processes, that occur naturally, facilitate memory consolidation and reconsolidation and it is during these processes, new long-term memories are formed or old ones updated and because of this, learning takes place.
Hypnosis, therapeutically speaking, is mostly voluntary, one can stay in it for an indefinite period, e.g. as determined by the type of therapy and/or the therapist and it can, under the guidance of a skilful hypnotist, reach depths that are mostly not possible during normal everyday trance states. Essentially, therapeutic hypnosis occurs when we are guided into these deeper sleep-like states (as in nighttime sleep, theta brainwaves), where we experience REM (rapid eye movement) and NonRem activity. NonREM occurs in phases 1. (twilight) 2. (dream states) and 3. (delta wave). NonREM 3 is slow-wave sleep and often called deep sleep. Because of time and space, this is a very brief description of this fascinating subject and one that may differ greatly from that of others in this field? Nevertheless, it is an opinion, based upon thousands of hours of research into the mechanisms of the brain and almost 20 years of clinical experience!
The most empowering effects of hypnosis most often occur in these deeper and slower states of brainwave activity. But any effect is caused by changes in the way our brain works. Hypnotists often say these neural changes occur in the subconscious mind but the mind is merely a hypothetical/philosophical construct, one that makes the output of brain function easier to explain and/or understand. During hypnosis, the hypnotist guides the client into deeper states of trance, a state that effectively communicates directly with the brain, primarily via consciousness and then through nonconscious auditory processes (what some refer to as the inner or subconscious mind) and therein, changes at a neurological level take place.

I believe the main reason there is so much confusion over what hypnosis really is, is because changes to life experience can occur in many different ways, e.g. through prayer, meditation, NLP, CBT/psychotherapy or counselling or even during moments of consciousness (will power)? In fact, a lot of the habits and behaviours we have, developed this way? So some interventions can clearly be seen as a consequence of being de-hypnotised! One could easily describe changes that occur by the above methods, as occurring in light or even non-trance states? However, when it comes to helping clients resolve more serious conditions, perhaps those that have developed over many years, often a consequence of things like trauma, cumulative trauma or neurological illness or malaise, or even mental/emotional illness, the deeper, more specific state of hypnosis is particularly efficient in creating solutions!
Many mental or emotional conditions arise in order to make life more tenable, relative to the period of life one is in, e.g. childhood, adolescence, adulthood etc. In response to its perception of life, the brain develops many dysfunctional coping strategies and/or defence mechanisms, which can evolve into what are sometimes termed hidden agendas. These strategies, mechanisms and agendas are stored deep within implicit, emotional, memory systems, systems that work outside of conscious awareness and therefore, do not require our conscious consent; i.e. they are automatic! Hypnosis helps reprogramme these automatic emotional memories in ways that allow us to recapture life and change these strategies. While the primary objective of this mental/emotional malaise is to help us survive, thus being protective in nature, the reality is, they make life a very scary place, e.g. during a panic attack! In treating these conditions, I have found deeper trance work often yields better, quicker and more permanent results. This is because the change required is made at deeper neurological levels while the brain is in the same stages of brainwave activity, the very brain states where these conditions often first manifest into long term memories and behavioural habits.
All change, at the level of behaviour, emotion and feeling, is consequential of changes in brain function. Everything we do that is deemed a habit or behavioural trait occurs through this process. So, it makes perfect sense that being in this same neural sleeplike state, we can reverse these maladapted mental states, change unwanted behaviours and develop new and more functional ways to live. While I say "sleeplike state" it is worth pointing out that hypnosis is similar to sleep but not exactly the same, it is the brainwave states that are similar.
When explaining what hypnosis is, some hypnotherapists say you will hear every word and this is not necessarily true, I question that if you heard every word; were you actually in hypnosis? After all, you don't even hear every word in a normal conversation, so why would it be any different in hypnosis? They would argue, that this is because hypnosis creates a heightened sense of awareness! Well, over many years as a clinical hypnotherapist, thousands of my clients have made profound changes in their life through hypnosis and they would argue against the above comment of "heightened awareness!" This is mostly because they say they had very little awareness of what I said to them, nevertheless, still it worked? Any heightened awareness is merely a consequence of the fact, that hearing cannot be switched off and what the brain hears it processes!
The brain converts spoken language into brain language via electrochemical processing and at deeper levels this allows the hypnotist to communicate directly (at a cellular level), via the auditory cortices, with the brain and this is what creates the profound changes we experience! One of the reasons this change occurs is because of another type of brainwave, PGO (ponto-geniculo-occipital) waves. PGO waves are short electrical bursts and offer us a peek into the deep neural mechanisms within the brain, that explain how the brain works during sleep states. PGO waves, which precede REM sleep, occur when we are in sleep states, the same place where we go in hypnosis. PGO waves occur throughout and across many of the key areas of the brain and are actually instrumental in learning and memory consolidation/reconsolidation. Once in REM sleep, an area of the brainstem called the locus coeruleus, almost completely shuts down norepinephrine production, the precursor of the stress response, essentially creating calmer brain states, thus allowing the brain to do its magic! Activity seen in the hippocampus during learning are then replicated during sleep/hypnosis and that is what hypnosis is!
